Office Policies and Waiver
Please be advised of the policies for this office. Your signature/booking signifies acceptance of these policies.
Clientele must wear face covering upon entry into the building and remain with their masks on unless otherwise stated, such as when lying face-down on the massage table or due to any medical condition (with physician's statement) that prevents the client from wearing a mask. Masks are a mandatory requirement per MA state laws surrounding safety for phase 2 businesses such as Massage Therapy. By signing this waiver you, client; agree to the statement in truth that you do not currently have any symptoms of Covid-19 and or with fever. Symptoms of Covid can be found here:
If you are with symptoms and or feeling ill, we advise you seek medical help/advice and quarantine, you will not be seen if you are ill. Clientele are provided with hand sanitizers on premise as well as hand washing with hot soap and water and social distancing is encouraged unless you are receiving treatment where social distancing is not possible.
24 hour notice for booking - no same day, by appointment during hours of operation only.
We understand that life can throw a wrench in your day and sometimes it's unpredictable. Therefore if you cannot make it to your scheduled session, we offer you to reschedule and still be able to enjoy the benefits of massage therapy. Should you know ahead that you cannot commit to your session due to unforeseen circumstances, please let us know early so we can reschedule you and schedule other clients also looking to enjoy massage and wellness. No cancelation fee.
Appointment times are as scheduled and cannot extend beyond the stated time to accommodate late arrivals. Please be on time for your appointment. If you do show up late you will only receive your remaining time until your scheduled end-time for your session. If you are more than 30 minutes over your booked time and do not plan on showing up to your appointment, you will be notified and given the option to reschedule, or schedule your next appointment at your convenience.
Massage/bodywork is not appropriate care for infectious or contagious illness. Please cancel your appointment and or reschedule as soon as you are aware of an infectious or contagious condition. We also do not encourage the spread of infectious disease or illness, if you are sick, stay home. Please see Coronavirus Update above.
Please be courteous, if you are able to shower and freshen up before your massage, it is encouraged and appreciated. There is no wearing of shoes inside of the office and treatment rooms at all times, this is for cleanliness and to help keep the space open and inviting for everyone to enjoy. Hand sanitizers are provided on premise for your convenience as well as common area bathrooms for handwashing if prefered. Thanks.
Conduct & Draping
Under no circumstances shall acting inappropriately be tolerated. Client is to act in a respectful manner and not engage in any sexual misconduct or negative behavior at any time. This will result in immediate termination of the treatment session and the appropriate action will be taken. Draping is done in a safe and secure manner, only the areas of your body that are being worked on will be exposed/undraped at any time. Any draping requests that are deemed unsafe or insecure, will not be honored. Thank you for your utmost understanding.
Release of Medical Records
Your signature/ online booking/ prepayment authorizes the release of all of your medical records on file in this office, for the purpose of processing your claims, to the following: your attorney, the healthcare providers attending to this condition, and the insurance case managers. Medical records will not be edited unless otherwise stated in an exclusive release of medical records signed through your attorney.
Offers and Promotions
Offers advertised whether in office or online are subject to change without notice, all offers will be honored within reason. In order for the most efficient use, clients must adhere to policies and be in good standing - with no outstanding balances or unresolved account issues.
Payments & Refunds
All payments can be submitted via the online scheduler in Massage Book, or in person at the Suite, All Major Credit or Debit cards are accepted. Cash payments are preferred (we do not carry any change). Please note that fees for financial transactions from applications and institutions such as CASHAPP and PAYPAL are typically anywhere from 2% to 10% of the fee. Client is responsible for the total cost of services, gratuity, including fees.
No Refunds. All Sales are Final including any retail products purchased by the client.
Gratuity is greatly appreciated but never expected. Recommended Gratuity for service is 20% typically for this type of professional service industry.
Monthly Memberships (Only if you hold a monthly membership this applies.)
Your entrance into an agreement for service is valid from the date of initiating membership until renewal, cancelation, or forfeiture. Warm Palms Massage's Three months, Six months, and 12 months memberships require a monthly payment (less gratuity)
Warm Palms will invoice you on the same day, whether it's the first of the month or the fifteenth of the month. Memberships entitles clients to a monthly service and must be scheduled every month, any unused portion of your membership will result in a forfeiture and losing out on assignment of benefits. Please see payments and refunds above. If you decide to end your membership - the remaining months due will be billed in accordance with your agreement.
I fully understand that Warm Palms Massage Therapy is not responsible for the potential problems with me during or after the treatment. With my present statement, I irrevocably sign claiming any damages against Warm Palms Massage Therapy- Warm Palms Massage Therapy will not be held liable for services provided. I also have read and agreed to all office policies stated herein, and understand that these office policies periodically are updated or can be changed at any time without notice to me. By booking an appointment for service and providing payment, I acknowledge that I am adhering to these governing office policies, terms and conditions.

The prices we charge for using our services / for our products are listed on the website. We reserve the right to change our prices for products displayed at any time, and to correct pricing errors that may inadvertently occur. We may, without prior notice, change the services; stop providing the services or any features of the services we offer; or create limits for the services. We may permanently or temporarily terminate or suspend access to the services without notice and liability for any reason, or for no reason.
The Service and all materials therein or transferred thereby, including, without limitation, software, images, text, graphics, logos, patents, trademarks, service marks, copyrights, photographs, audio, videos, music and all Intellectual Property Rights related thereto, are the exclusive property of [>>Mr. Simr<<].
